Important Forms

To ensure every cremation is carried out without delays and in compliance with your wishes and the law, we need certain information and your authorization to proceed.

Below are the relevant legal forms. Please complete and sign these forms after you have arranged the cremation, then return them to us at Cremation Society as soon as possible. Without them, we cannot carry out the cremation.

How to return the forms

Once you have completed the forms, you can scan them and email them to us at info@cremationsociety.com, or mail them to us at PO Box 1179, Romoland, CA 92585 using first class or registered mail.

This form allows us to file the death certificate and other required permits from the county health department. Return to us as soon as possible to comply with Californian law

This form allows us to obtain the death certificate and other required permits from the county health department. This document must be printed and then signed in front of a witness. Return to us as soon as possible.

Riverside County Coroner Request for Release

This form is required by the Riverside County Sheriff-Coroner’s office to release your loved one from their office to our care. This form is only needed for deaths occurring in Riverside County with your loved one located at the Coroner’s facility.

This form memorializes if there was a pre-need funeral agreement on file with our firm for your loved one. Return to us as soon as possible to complete the arrangement file.

This form is required by the San Diego County Medical Examiner’s Office to release your loved one from their office to our care. This form is only needed for deaths occurring in San Diego County with your loved one located at the Medical Examiner’s Office.
